
Terms of Agreement and Consent
Communication
Method of Communication: I understand and agree that Unity College uses the @unity.edu e-mail assigned to me as the official method of communication with me, and I am responsible for reading my e-mails I receive from Unity College on a timely basis. Prior to the creation and assignment of an @unity.edu e-mail address, I consent to and authorize Unity College communicating through the personal e-mail address on record, and understand and agree that this is the official method of communication with me until an @unity.edu e-mail address is established and assigned. If you do not want to receive Financial Aid communications electronically, please contact us at 1-833-UnityGo to Opt-Out.
Updating Contact Information: I understand and agree that I am responsible for keeping Unity College records up to date with my current physical mailing addresses, email addresses, and phone numbers within my student portal. Upon leaving Unity College for any reason, it is my responsibility to provide Unity College with updated contact information for purposes of continued communication regarding any amounts that remain due and owing Unity College.
Transferring College Credits
I understand that college credits may only be transferred to Unity College through the submission of an Official College transcript. Official transcripts are received from an authorized third party or directly from another institution. If I fail to provide Official Transcripts to Unity College, transfer credit will not be awarded and I may be enrolled in Unity College courses that would have otherwise been fulfilled through transfer at time of enrollment. I also understand that the Department of Education, may without notice, require us to obtain your official transcripts from you for financial aid eligibility.
Transcript Request Notice
Students who have completed high school at least 3 years prior to applying to Unity College’s Hybrid Learning program are not required to provide proof of high school completion unless requested by the Department of Education as part of the financial aid process.
Billing
Method of Billing: I understand that Unity College uses electronic billing (student portal) as its official billing method and therefore I am responsible for viewing and paying my student account bill by the scheduled due date. I further understand that failure to review my bill does not constitute a valid reason for not paying my bill on time. I also understand that if I am a qualified individual in need of an ADA accommodation for electronic billing, I should contact the Student Accounts Office.
Late Payment Charge: I understand and agree that if I fail to pay my student account bill or any monies due and owing Unity College by the scheduled due date, Unity College may assess a finance charges at the rate of 1.5% per month on the past due portion of my student account until my past due account is paid in full.
Business Hold: I understand and agree that if I fail to pay my student account bill or any monies due and owing Unity College by the scheduled due date(s), Unity College may place a business hold on my student account, preventing me from registering for future classes, requesting transcripts, viewing grades or receiving my diploma.
Collection Agency Fees: I understand that if I fail to pay my student account bill or any monies due and owing Unity College by the scheduled due date(s), and fail to make acceptable payment arrangements to bring my account current, Unity College may refer my delinquent account to a collection agency. I further understand that I am responsible for paying the collection agency fee which may be based on a percentage at a maximum of 33.3% percent of my delinquent account, together with all costs and expenses, including reasonable attorney’s fee, necessary for the collection of my delinquent account. Finally, I understand that my delinquent account may be reported to one or more of the national credit bureaus.
Returned Payments/Failed Agreements: If a payment made to my student account is returned by the bank for any reason, I agree to repay the original amount of the payment plus a returned payment fee of $25.00. I understand that multiple returned payments and/or failure to comply with the terms of the payment plan or agreement I sign with Unity College may result in cancellation of my classes and/or suspension of my eligibility to register for future classes at Unity College.
Billing Errors: I understand that administrative, clerical or technical billing errors do not absolve me of my financial responsibility to pay the correct amount of tuition, fees and other associated financial obligations assessed, as a result of my registration at Unity College. If you wish to dispute any or all charges, you must contact the Student Accounts Office at 207-509-7261.
Withdrawal
I understand and agree that if I drop or withdraw from some or all the classes for which I
register, I will be responsible for paying all or a portion of tuition and fees in accordance with the published tuition refund schedule in the Unity College Catalog. I have read the terms and conditions of the published tuition refund schedule and understand those terms are incorporated herein by reference. I further understand that my failure to attend class or receive a bill does not absolve me of my financial responsibility as described above. If I decide to completely withdraw from Unity College, I will follow the instructions in the Unity College Catalog, which I understand and agree are incorporated herein by reference.
Privacy Rights and Responsibilities
I understand that Unity College is bound by the Family Education Rights and Privacy Act (FERPA) which prohibits Unity College from releasing information from my education record without my written permission or as otherwise allowed by law. Therefore, I understand that if I want Unity College to share my information from my education record with someone else, I must provide written permission by the procedure outlined in the Unity College Catalog. I further understand that I may revoke my permission at any time as instructed in the same procedure.
IRS Form 1098-T
I agree to provide my Social Security number (SSN) or taxpayer identification number (TIN) to Unity College upon request as required by Internal Revenue Service (IRS) regulations for Form 1098-T reporting purposes. If I fail to provide my SSN or TIN to Unity College, I agree to pay any and all IRS fines assessed as a result of missing SSN/TIN. I consent to receive my annual IRS Form 1098-T, Tuition Statement, electronically from Unity College. I understand that if I do not consent to receive my FORM 1098-T electronically, a paper copy will be provided. I understand that I can withdraw this consent or request a paper copy by contacting the Student Accounts Office.
Student Age
I understand and agree that if I am younger than the applicable age of majority when I execute this agreement, that the agreement that the educational services provided by Unity College are a necessity, and I am contractually obligated pursuant to the ‘doctrine of necessaries.
Entire Agreement
With respect to the matters described herein, this agreement supersedes all prior understandings, representations, negotiations, and correspondence between the student and Unity College. This constitutes the entire agreement between the parties, and shall not be modified or affected by any course of dealing or course of performance. This agreement may only be modified by Unity College if the modification is in writing and is signed by me, the student. Any modification is specifically limited to only those policies and/or terms addressed in said signed modification.
