In addition, learners will need to provide diagnostic documentation from their health care provider by submitting a Health Care Practitioner Form. Students may also submit recent academic documentation, such as an IEP or 504 Plan, to provide the Coordinator with context for past modifications and accommodations. These materials will be assessed, eligibility will be determined, and the Coordinator will contact the Learner directly to discuss any appropriate accommodations and strategies.
